By completing a new prescription submission form, you agree to provide a pharmacist access to your personal health information. Furthermore, you agree that the pharmacist may reveal all prescription and personal health information to physician(s) and drug plans as needed to maintain the quality of care.

If a third party is picking up a refill on the patient's behalf, identification may be required. Once your medication is ready, the pharmacy will call you.

Woodgreen Pharmacy cannot be held accountable for requests that are delayed or lost due to technological malfunction.
